Shungu, Natelson, Sterwart en kollega's hebben, om onderstaand verklaringsmodel te testen,

de hersenen van 15 ME/CVS-patiënten, 15 mensen met depressie en 13 gezonde mensen

onderworpen aan magnetische resonantie-onderzoek (1H MRI en MRSI).

 

 

 

 

Onderwerp van onderzoek waren

  • de bloedtoevoer naar de hersenen,
  • glutathione, als een graadmeter voor de concentraties antioxidanten in de hersenen,
  • lactaat ("melkzuur"), als een maatstaf voor de anaerobe ("zuurstofloze") energieaanmaak,
  • de mitochondriale functie (werking van de energiecentrales) die mogelijk verstoord is,
  • de symptomen en de samenhang van de symptomen met bovenstaande graadmeters.

 

 

 

De conclusies uit hun onderzoek, kort samengevat:

  • ME/CVS-patiënten hebben aanzienlijk meer last van pijn en fysieke beperkingen,
  • de mensen met depressie hebben veel grote problemen met het "emotioneel welzijn",

    terwijl ME/CVS-patiënten op die gebieden nauwelijks afwijken van gezonde mensen.

  • De bloedtoevoer naar bepaalde hersengebieden bij ME/CVS-patiënten is minder.
  • De concentratie lactaat in de hersenen is sterk verhoogd in ME/CVS (en depressie).
  • Dit was reeds eerder door Shungu et al. in studies aangetoond: klik hier en hier.

  • De concentraties glutathione zijn beduidend minder in ME/CVS en depressie.
  • De lagere concentraties glutathione zijn gerelateerd aan hogere concentraties lactaat.
  • Over alle groepen heen, is er een relatie tussen (de ernst van) van diverse symptomen
  • en de lagere concentraties glutathione en hogere concentraties lactaat,

    hetgeen niet zo verwonderlijk is: hersenen hebben nu eenmaal zuurstof nodig.

  • Mitochondriale dysfunctie in de hersenen van ME/CVS-patiënten werd niet gevonden.
  • Verminderde werkzaamheid van de mitochondria is eerder door Myhill c.s. vastgesteld.

    De onderzoekers stellen dat de gebruikte methodiek wellicht niet toereikend is. 

De steekproef was te klein was om een aantal aanvullende konklusies te kunnen trekken,

bijvoorbeeld over de "tussenpositie" van mensen met depressie t.a.v. glutathione en lactaat.

 

Tot slotte nog dit, om mogelijke "misverstanden" uit de wereld te helpen:

de gemiddelde score van mensen met ME/CVS m.b.t. verwaarlozing en misbruik

lag lager dan die van de gezonde mensen. Die van mensen met depressie lag hoger.

Jammer genoeg was de onderzoeksgroep te klein om "toeval" uit te sluiten.

 

 


 

 

 

Increased ventricular lactate in chronic fatigue syndrome. III. Relationships to cortical glutathione and clinical symptoms implicate oxidative stress in disorder pathophysiology.

Abstract

 

Chronic fatigue syndrome (CFS) is a complex illness,

which is often misdiagnosed as a psychiatric illness.

 

In two previous reports, using (1) H MRSI, we found

significantly higher levels of ventricular cerebrospinal fluid (CSF) lactate

in patients with CFS

relative to those with generalized anxiety disorder and healthy volunteers (HV),

but not relative to those with major depressive disorder (MDD).

 

In this third independent cross-sectional neuroimaging study,

we investigated a pathophysiological model

which postulated that

elevations of CSF lactate in patients with CFS

might be caused by

increased oxidative stress,

cerebral hypoperfusion and/or

secondary mitochondrial dysfunction.

 

Fifteen patients with CFS,

15 with MDD and

13 HVs

were studied

using the following modalities:

  1. (1) H MRSI to measure CSF lactate;
  2. single-voxel (1) H MRS to measure levels of cortical glutathione (GSH)
  3. as a marker of antioxidant capacity;

  4. arterial spin labeling (ASL) MRI to measure regional cerebral blood flow (rCBF); and
  5. (31) P MRSI to measure brain high-energy phosphates
  6. as objective indices of mitochondrial dysfunction.

We found

elevated ventricular lactate and decreased GSH

in patients with CFS and MDD

relative to HVs.

 

GSH did not differ significantly between the two patient groups.

 

In addition, we found

lower rCBF in the left anterior cingulate cortex and the right lingual gyrus

in patients with CFS relative to HVs,

but rCBF did not differ between those with CFS and MDD.

 

We found

no differences between the three groups

in terms of any high-energy phosphate metabolites.

 

In exploratory correlation analyses, we found that

levels of ventricular lactate and cortical GSH

were inversely correlated, and significantly associated with

several key indices of physical health and disability.

 

Collectively,

the results of this third independent study support

a pathophysiological model of CFS

in which increased oxidative stress may play a key role

in CFS etiopathophysiology.
