ME en CVS onverklaarbaar?

 

Het 4I-model:

een plausibele verklaring

voor karakteristieke symptomen

op basis van

immunologische afwijkingen

 

 

 

 


 

 

 

Met dankzegging aan alle echte deskundigen, o.a. prof. de Meirleir en prof. Pall, heb ik

een artikel geschreven waarin ik de eindjes (verklaringsmodellen) aan elkaar probeer te knopen.

 

In het 4I verklaringsmodel (zie onder) spelen vier immunologische afwijkingen in ME/CVS

(inflammatie, Th2-dominante immuunactivatie, immuunsuppressie en immuundysfunctie)

en hun directe gevolgen, met name oxidatieve en nitrosatieve stress, een hoofdrol.

 

Die immunologische afwijkingen worden (deels) in stand gehouden door (chronische en/of

reactiverende) infecties, darmontstekingen/"lekke darm" en "neo-epitopen" (zie onder).

Oxidatieve en nitrosatieve stress

  • remmen de oxidatieve fosforylering (aerobe energieproductie) af,
  • brengen schade toe aan de mitochondria en
  • lichaamseigen eiwitten die veranderen in "neo-epitopen": lichaamsvreemde eiwitten,
  • dragen bij aan "hypersensitiviteit": overgevoeligheid voor pijn, geluid, geur en licht, en
  • verstoren de bloedcirculatie, mede omdat stikstofoxide een rol speelt in de vasodilatatie.

Immuunactivatie en oxidatieve stress remmen de werking van de bijnieren (HPA-as).

 

Verhoogde concentraties cytokines worden weerspiegeld in de hersenen

en leiden tot wijd verspreide spier-/gewichtspijn en "een koortsachtig gevoel".

 

Het 4I-model omvat vier vicieuze cirkels:

  1. infecties en immuunsuppressie/immuundysfunctie, mogelijk resulterend in nieuwe infecties.
  2. de ONOO-cyclus van prof. Martin Pall (superoxide + stikstofoxide = peroxynitriet)
  3. oxidatieve/nitrosatieve stress resulteert in neo-epitopen die het afweersysteem activeren,
  4. waardoor (extra) oxidatieve en nitrosatieve stress ontstaat.

  5. versterkte terugkoppeling van cytokines en oxidatieve/nitrosatieve stress naar de HPA-as.

Inspanning doet de immunologische afwijkingen en oxidatieve en nitrosatieve stress toenemen,

hetgeen mogelijkerwijs een verklaring vormt voor post-exertionele malaise (klik hier).

 

Het I4-model vertoont een aantal overeenkomsten met het neuro-immune model van Morris/Maes,

maar er zijn ook een aantal belangrijke verschillen tussen de twee verklaringsmodellen.

 

De vicieuze cirkel die aan de basis van het 4I-model staat, ontbreekt in het Morris-en-Maes-model.

Infecties zijn in het neuro-immune model van Morris-en-Maes wel een oorzaak, maar,

net als in het (bio)psychosociale model van Vercoulen, géén (relevante) instandhoudende factoren.

 

Het 4I-model is uiteraard niet volledig en zal gelet op de definitie van CVS

nooit in zijn geheel op elke CVS-patiënt van toepassing zijn,

maar toont wel aan dat ME en CVS géén "medische onverklaarbare aandoeningen" zijn.

 

Het is hopelijk een aanzet tot een inhoudelijke discussie over

kenmerkende fysiologische afwijkingen in ME/CVS en

hun rol in de verklaring van symptomen, m.n. "post-exertionele malaise".

Characteristic symptoms of Myalgic Encephalomyelitis (ME) are

(muscle) weakness, muscle pain, cognitive deficits, neurological abnormalities,

but above all post-exertional malaise: a long-lasting increase of symptoms after a minor exertion.

 

In contrast, Chronic Fatigue Syndrome (CFS) is primarily defined by "chronic fatigue".

 

Since "chronic fatigue" is not mandatory for the diagnosis ME, and

post-exertional malaise and cognitive deficits are not obligatory for the diagnosis CFS,

the case criteria for ME and CFS define two distinct, partly overlapping nosological entities.

 

ME and CFS are considered to be enigmatic diseases,

qualified by some authors as "medically unexplained syndromes" or "functional syndromes".

 

However,

specific abnormalities consistently observed over the years and their direct and indirect sequels

can plausibly explain characteristic symptoms, e.g. "exhaustion" and pain.

 

Abnormalities established repetitively incorporate

  • immunological aberrations
  • (inflammation, immune activation, immunosuppression, and immune dysfunction),

  • persistent and/or reactivating infections,
  • gastro-intestinal dysbiosis,
  • oxidative and nitrosative stress,
  • mitochondrial dysfunction,
  • a (prolonged) deviant response to exertion and orthostatic stress,
  • circulatory deficits, and
  • neurological abnormalities.

This article depicts the 4I hypothesis,

an explanatory model for ME (CFS)

with a central role for

four types of immunological abnormalities:

  1. inflammation,
  2. (Th2-predominated) immune activation,
  3. immunosuppression, and
  4. immune dysfunction.

The potential direct sequels of these abnormalities,

e.g. increased oxidative and nitrosative stress, (reactivating or chronic) infections,

and their possible indirect consequences,

e.g. mitochondrial dysfunction, hypothalamic-pituitary-adrenal axis (HPA) axis hypofunction, and

cardiovascular dysregulation,

can plausibly explain various distinctive symptoms of ME/CFS,

e.g. "exhaustion", (muscle) weakness, pain, cognitive deficits,

a flu-like feeling, and post-exertional "malaise".
